Appointments at Churchfields

We are still mainly offering on the day telephone consultations but may offer a telephone consultation for a later date if this is appropriate. We appreciate that it can be very beneficial to speak to the same doctor or physician associate and by allowing some forward booking we would hope to be able to offer this. Direct Access External Services

Test Results

Test results can either be obtained via telephone or you can request access to your results online via the NHS app or Patient Access. To enable this please come into surgery with proof of ID and then we can set you up. Please click here to find out more information about test results.

Fit Notes and other Admin requests

You can request a Fit Note and make other non emergency administration requests via our AccuRx service
