Register as a New Patient
The Doctors welcome new patients located within our catchment area.
As it often takes some time for records to reach us from your previous GP, all newly registered patients will be asked to complete a health questionnaire. We offer our new patients a check up with our healthcare assistants, as well as a routine consultation with one of our doctors if required.
Whilst your documents might not reach us right away, we offer medical treatment from the date of registration. Patients choosing to move from a local practice to us will be registered with us from the day of their first routine consulation with a Doctor.

Requirements for registrations:
In order to register you on our system, we require the following:
- If you are over 16 you will need to fill in the "New Patient Registration Form for OVER 16s" which can be found below. We will also need a photo ID and proof of address. This can be:
One document of identification, such as a passport or driving licence.
One document that verifies your proof of address, which can be a utility bill, bank or credit card statement or driving licence (provided it shows your current address and has not been used as the ID document. - If you are under the age of 16 then you will need to fill in the "New Patient Registration for UNDER 16 years of age" which can also be found below.
- If you are registering a new born baby, please send us a copy of the baby's red book. This should include the baby's NHS number. Please send this to churchfields.surgery@nhs.net

Temporary Patient Registrations
If you are ill whilst away from home, or not registered with a Doctor but need to see one, you can receive emergency treatment from a local GP practice for up to 14 days. After this time, you would be required to register as a temporary or permanent patient at that practice.
You can be registered as a temporary patient for up to three months. This allows you to stay on local GP practice list, whilst still remaining on the register for your permanent GP. After three months, you will have to re-register as a temporary patient, or permanently register with the local practice.
To register as a temporary patient, simply contact the local practice that you wish to register for. Practices are not obligated to accept you as a temporary patient, though they are required to offer emergency treatment when requested. You cannot be registered as a temporary patient at a local practice in a town/area where you are already registered.

There is easy disabled access throughout our building here at BHI Parkside.
From the disabled parking spaces located near our main entrance, through to the automatic doors, leading to a pair of lifts for reaching the upper levels of our site, we aim to make your visit a comfortable one.
Our Receptionists are always happy to assist you, and we have a hearing loop in place for those who require it. Our main Reception desk features a reduced height counter, as well as a private discussion room if there is something you wish to discuss in confidence.
Our Reception areas feature a variety of seating environments. We also provide free Wi-Fi access. If you are having any difficulty accessing or services, please do ask our Receptionists who will do all they can to offer appropriate assistance.
