We would like to know how we can improve our service to you and how you perceive our surgery and staff. 

To help us with this, we are setting up a virtual patient representation group so that you can have your say. We will ask the members of this representative group some questions from time to time, such as what you think about our opening times or the quality of the care or service you received. We will contact you via email and keep our surveys succinct so it shouldn’t take too much of your time.

We aim to gather around a hundred patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.

What will it involve?

We will ask you to carry out a short online survey into what you feel are the priorities for the practice.  We will then design a survey with this in mind and ask you to look at it.  It will then be completed by other patients and we will share the results with you and discuss and agree changes.  On average we will only contact you no more than once per month.  We will do one survey by the end of March 2012 and a further survey later the same year. 

Will the questions you ask me be medical or personal?

We will only ask general questions about the practice, such as short questionnaires.

Who else will be able to access my contact details?

Your contact details will be kept safely and securely and will only be used for this purpose and will not be shared with anyone else. Doctors will only see general feedback if they are responsible for making changes.

Will the surveys be anonymous?

Yes

What if I no longer wish to be on the contact list or I leave the surgery?

We will ask you to let us know by email if you do not wish to receive further messages.
